Hospice of St. Lawrence Valley has provided compassionate, expert end-of-life care to the residents of St. Lawrence County since 1983.

While the ways in which this care is provided have changed over the years, the staff’s unwavering commitment to patients and families has remained steadfast.

Knowing exactly how, when, and where Hospice provides support empowers patients and families to make well-timed decisions about starting end-of-life care.

When Hospice helps

Hospice provides end-of-life care to patients with a prognosis of six months or less if the disease follows its normal course. This does not necessarily mean a patient will pass away within six months, but it indicates that they are likely in the final stage of their illness.

Some patients receive hospice support for only a few days or weeks. Even so, they and their families receive the same high-quality, skilled end-of-life care.

Patients who are referred to hospice earlier can benefit from the full range of services for a longer period, allowing them and their families to build relationships with the care team.

If a patient or physician is unsure whether it is the right time for hospice, they are always encouraged to call for more information.

Where Hospice helps

Hospice of St. Lawrence Valley provides this specialized care wherever the patient calls home.

All team members travel directly to the patient’s bedside to deliver compassionate, expert care.

While there is no hospice residence in St. Lawrence County, patients still receive exceptional end-of-life care in the comfort and familiarity of their own homes.
