Four St. Lawrence County organizations getting $1.6 millon for healthcare projects

Four St. Lawrence County organizations are splitting $1,643,000 in grants to support healthcare projects.

St. Lawrence Health System, which operates Canton-Potsdam and Gouverneur hospitals, will receive three grants.

Canton Enriched Housing (CEH) will get $235,400 for a mid-level provider that will “allow preventative measures to reduce unnecessary emergency department visits,” according to Adirondack Health Institute, which announced the funding. They say CEH residents are “heavy utilizers of the emergency department.”

SLHS will also get $350,000 for “a comprehensive approach to the accurate identification of COPD patients and post-acute coordination so as to prevent readmissions and improve patient quality of life,” AHI said.

SLHS will get an additional $245,323 for behavioral health services in their Potsdam Primary Care clinic.

Hospice and Palliative Care of St. Lawrence Valley will get $200,000 aimed at decreasing emergency department visits and hospitalizations of Medicaid patients “by functionally integrating home health and palliative care services,” AHI said.

United Helpers Riverledge and Maplewood campuses will get $235,370 to “improve the clinical diagnostic capabilities of each campus so as to prevent unnecessary hospitalizations and outpatient visits,” AHI said.

United Helpers Mosaic will get $377,000 for the Pillar Program. AHI said the program is designed “to break the cycle of disability and dependence and move toward independence for some of NY’s neediest youth and their families.”

The funding comes from the state’s Delivery System Reform Incentive Payment (DSRIP) program’s efforts to fundamentally restructure the Medicaid system, according to AHI.
